QUALIFICATIONS: 1. G raduate of an approved school of nursing with current registration in Massachusetts. BSN required. 2. M inimum two years of experience (within the last 5 years) with renal medicine-focused patient care required. 3. M inimum two years of experience (within the last 5 years) with abdominal surgery or transplant-focused patient care required. 4. T riage experience (telephone-based clinical assessment technique) required. 5. C omprehension with the ability to educate patients regarding immunosuppressants, rejection, and infection treatments strongly preferred. 6. P rior experience (1+ years) as a Clinical Transplant Coordinator strongly preferred. 7. F amiliarity with complex medication regimens and management preferred. 8. R ecent experience (within 3 years) in ambulatory nursing preferred. 9. ABTC C ertified Clinical Transplant Coordinator preferred. Skills, Competencies, and Abilities: 0. STAFF NURSE -RENAL/ PANCREAS TRANSPLANT - COORDINATOR/ 40 Hours/ Variable/ Rotating Shift/ BWH Boston / Ambulatory nursing Excellent Care to Patients and Families • The Best Staff • In the Safest Environment The Transplant coordinator will primarily serve in Post - Renal / Pancreas Transplant, caring for kidney or pancreas transplant recipients. The coordinator will need an understanding of both pre - and post-renal / pancreas transplant care. Based on department needs, care may extend to living kidney donor patients, pre-transplant potential candidates, and waitlisted candidates. The nurse coordinator schedule will vary weekdays, based on patient care needs and when functioning successfully independently. Post-Transplant: - Accountable for assessing, planning, implementing and evaluating a plan of care for our post operative renal and pancreas transplant patient population. - Provides in-patient teaching for renal and pancreas transplant recipients and support person/caregiver,coordinated with the inpatient nursing staff to ensure consistency and collaborates with the multidisciplinary inpatient transplant team rounds. - Provides ongoing education to transplant patients and family members regarding important aspects of care, including but not limited to activity, diet, medication adherence and healthcare follow-up. Helps to maintain current supply of patient/family education materials. - Responsible for medical management care coordination for patients in collaboration with Transplant Nephrologists and Surgeons. Responsible for revising plan of care as needed. - Attends post-transplant clinic sessions and provides appropriate teaching, emotional support, medication administration, wound care, drain care, and other duties as needed. - Coordinates renal transplant medicine clinic visit follow-up, including review of same day and faxed labs results with transplant nephrologist, leading to a revised care plan. - Facilitates interactions of the multidisciplinary team members, including physicians, social work, finance coordinator, transplant pharmacist, nutritionist and outside consultants. - Telephone contact with patients to assess clinical status, monitor response to therapy, adjust medications in consultation with clinical providers, and organize outpatient or inpatient follow-up as needed. - Working closely with transplant nephrologists, coordinates all outpatient care follow-up for post-transplant patients, including but not limited to prescription refills and medication changes, assisting/obtaining prior authorizations, arranging for testing and treatment services, and facilitating admissions as necessary within Brigham & Women’s or a patient’s local treatment facility. - Participate in interdisciplinary post-transplant meetings, provide current information regarding patients, relaying any new developments to the multi-disciplinary team, which may impact the care plan. - Sets priorities when organizing care for patients with varying acuity. - Maintain continuity of care through clear and concise (verbal and written) communication. - Other duties as assigned. Pre-Transplant: - Effectively manage patients through referral, evaluation, and waitlist phases toward transplant. - Meet potential transplant recipients and living donors (patients) in the clinic setting. Provide an overview of transplantation. - Communicate with living donor or recipient's PCP, referring nephrologist or endocrinologist, and other specialists as applicable to request records or coordinate testing and further care. - Maintain records on potential recipients. Professional Attributes - Establishes effective professional relationships with other members of the healthcare team. - Serves as role model and resource for other staff, providing assistance with the care coordination process, clinical skills and patient teaching. - Maintains communication with other organ transplant programs at Brigham & Women’s and other transplant centers, as needed, in order to keep abreast of current information. - Maintains professional relationships with appropriate professional organizations. - Regularly incorporates new scientific findings in specific area of expertise into clinical practice. Maintains awareness of new developments in the specialized area and participates in research activities. Participatory Management - Utilizes time and materials effectively. - Participates in the orientation of new staff members, students and others as appropriate. Provides input into the evaluation of other staff as appropriate. - Adheres to hospital and departmental policies.
