Current Registered Nurse licensure in the State of Massachusetts Baccalaureate degree from approved Nursing School Program preferred 3 years minimum in oncology practice is required, ambulatory experience preferred Excellent interpersonal and verbal/written communication skills Ability to problem-solve, function as a resource to other members of the multidisciplinary care team, and resolve complex issues on behalf of providers and patients. Demonstrates proficiency in a practice environment that promotes patient-centered care and willingness to advocate for patients and families. Displays e xceptional organizational skills and flexibility to manage multiple tasks and adapt to quickly changing situations. Possesses k nowledge of computer applications necessary to function in an ambulatory/outpatient setting. EPIC proficiency strongly preferred. Demonstrates flexibility to work seamlessly within different specialty clinics. The Mass General Cancer Center leads the way in delivering specialized and coordinated cancer care, with a team of over 500 cancer specialists and more than 30 multidisciplinary disease centers and programs. The Practice Nurse plays a vital role, working with a multidisciplinary team to provide comprehensive patient care, covering prevention, early detection, diagnosis, treatment, follow-up, palliation, bereavement, and survivorship. The Practice Nurse is key to coordinating patient care across specialized clinical centers and programs, including partnerships with radiation oncology, surgery, and clinical research, to develop and implement personalized care plans aimed at achieving optimal clinical outcomes. In the ambulatory/outpatient setting, the Practice Nurse systematically collects and assesses patient health data to develop, implement, and evaluate care plans. They perform assessments, coordinate treatments, and communicate with the multidisciplinary team. Additionally, the Practice Nurse provides education for patients and their loved ones, offering psychosocial and emotional support. They ensure care is holistic, safe, efficient, effective, timely, and adaptable to the cultural, spiritual, and ethnic needs of patients and their loved ones. The Practice Nurse serves as a contact with patients at home and in the ambulatory setting to ensure optimal clinical management in collaboration with attending physicians, sophisticated practice providers, fellow nurses, social work, support staff, and other members of the multidisciplinary care team. - Communicates with patients via daily telephone triage and through Patient Gateway messaging. - Collaborates with triage RN, MD, and APP for symptom management and medical urgencies/emergencies, guiding the patient to the appropriate level of care in the appropriate time frame. - Coordinates home services including, but not limited to, visiting nursing, palliative care and hospice, home infusion, laboratory services and monitoring, etc. - Educates patient and loved ones about disease, treatment plan, medications, and home care needs. - Provides assistance with disability forms, FMLA forms, Advanced Directives, and pre-authorizations as needed. - Assesses psychosocial needs of patients and loved ones. Collaborates with social services or psych for follow-up.
