Multiple Openings in Boston North & Boston South | Full-Time | M–F, 8–4 or 9–5

We’re hiring 6 Field Assessment RNs to join our mission-driven team—3 in Boston North, and 3 in Boston South!

About the Role

As an Assessment RN, you’ll work in the field conducting clinical assessments, gathering vital data to help develop individualized care plans for our members.

Territory:

• Boston North: Downtown Boston to Malden
• Boston South: Downtown Boston to Dorchester (I-90 to Dorchester)
• Start your day from the local Boston office—travel to members’ homes and communities
• Mileage reimbursement provided

✅ What You’ll Do

• Conduct in-person member assessments using MDS and functional screening tools
• Assess medication adherence, vital signs, and provide disease-specific education
• Identify unmet needs and escalate urgent issues
• Collaborate closely with interdisciplinary care teams
• Use EHR systems to document visits and coordinate care
• Adapt quickly to shifting priorities and evolving member needs

What You Bring

• Associate’s degree in Nursing (required)
• Active RN license in Massachusetts, in good standing
• 1+ year of clinical experience (including clinical rotations)
• MDS or Oasis experience
• Strong organizational and communication skills
• Comfortable working independently in the field
