Shift: Days – 7:00 AM to 3:00 PM (8-hour shifts)

Weekly Pay: $2,702.00/week

Job Description:

We are currently seeking a compassionate and experienced RN Case Manager for a 13-week assignment in Melrose, MA. This role involves coordinating patient care, supporting discharge planning, and advocating for patients throughout the continuum of care.

Key Responsibilities:

• Manage and coordinate patient care plans to ensure optimal outcomes.
• Collaborate with physicians, interdisciplinary teams, patients, and families.
• Assess patient needs, develop care plans, and facilitate timely discharges.
• Monitor the effectiveness of interventions and revise care plans as needed.
• Ensure compliance with healthcare regulations, policies, and documentation standards.
• Provide education and support to patients and families regarding care and resources.

Requirements:

• Active Registered Nurse (RN) license in Massachusetts.
• Minimum of 2 years of recent experience in Case Management.
• Current BLS certification (AHA required).
• Knowledge of discharge planning, insurance processes, and resource coordination.
• Strong clinical assessment, communication, and critical thinking skills.
• Experience with EMR systems (EPIC preferred but not required).

Contract Details:

• Assignment Length: 13 weeks
• Start Date: July 21, 2025
• End Date: October 20, 2025
• Shift: Day – 7:00 AM to 3:00 PM (Monday to Friday)
• Compensation: $2,702.00/week, paid weekly
