RN Case Manager

StartDate: 6/30/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

Minimum Required Qualifications –

  • MA RN license
  • BLS
  • 2 years of case management or utilization management experience and 3 years of medical/surgical nursing care experience.
  • Basic computer and typing skills.
  • Effective verbal communication skills and problem solving and conflict resolution skills.
  • Demonstrated ability to organize and work independently.

PREFERRED QUALIFICATIONS – Bachelor's Degree

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Usually M-F 8-4:30. Occasional weekend may be required.

START DATE – ASAP
