Position Description

Job Description: Registered Nurse (RN) - Home Health

Location: Boston, MA

Start Date: May 4, 2025

End Date: August 2, 2025

Shift: 8:00 AM - 4:30 PM (40 hours/week)

Pay: $2,850/week (Traveler) | $60/hour (Local)

Job Summary:

As a Home Health RN, you will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care will be delivered in alignment with the organization’s philosophy, policies, and community health practice standards. This role includes admission processes and case management duties, with thorough electronic documentation and interdisciplinary communication.

Responsibilities:

  • Assess patient conditions and develop individualized care plans
  • Provide skilled nursing care and patient education
  • Perform admission processes and case management responsibilities
  • Document care accurately in electronic health records
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Participate in weekend rotations (once every 6 weeks)

Requirements:

  • Active RN license
  • BLS certification
  • 1 year Certified Home Care experience
  • 1 year Case Management experience
  • Ability to travel to assigned cases
